• complete all intake forms

• discuss quitting date

• discuss withdrawal symptoms

• discuss coping strategies

• discuss five keys to quitting and how to implement

• overview of NRT, QuitKey, or other FDA approved products for tobacco cessation

• questions and answers - time permitting

Second session (individual)

• affirm quitting date

• review tobacco use log

• prescribe appropriate NRT, QuitKey, or other FDA approved products for tobacco cessation

Follow-up via phone or in person (individual)

• 30 Day

• 90 Day

• 180 Day

COST of program

• $75.00 per individual for first session

• $25.00 30 Day Follow-Up

• $50.00 90 Day Follow-Up

• $50.00 180 Day Follow-Up

• Cost of products range from $35.00 to approximately $200.00
